Responsibilities:- Conduct comprehensive health assessments and develop personalized care plans for patients with complex chronic illnesses
- Manage complex patients, focusing on reducing hospitalizations and improving health outcomes
- Collaborate with the healthcare team to coordinate and facilitate quality, consistent, cost-effective care
- Work autonomously and in collaboration with the healthcare team to achieve patient-centered care goals
- Minimum of two (2) years' experience as a Nurse Practitioner
- Current unrestricted Compact NP license to practice in the state
- Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
- Intermediate computer skills and proficiency in MS Word, Excel & Outlook required
- Preferred experience working in Value-Based Care or Population Health
- 5+ years' experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology or Case Management
- Experience in managing chronic diseases (CHF, diabetes, hypertension, diabetes)
