About the Role:

This is a unique opportunity to make a meaningful difference in the lives of terminally ill patients and their families. As a Hospice Registered Nurse, you will provide end-of-life care, assess patient needs, and develop personalized plans to ensure comfort and dignity.

Responsibilities:
  • Travel to multiple clients at the Hospice Facility and home settings, providing care and support as needed.
  • Chart patient information using Home Care Home Base HCHB.
  • Conduct physical and psychosocial assessments, developing written plans of care for each patient.
  • Provide pain management and symptom management, ensuring patients remain comfortable.
  • Collaborate with primary care physicians to address patient needs and concerns.
  • Supervise Hospice Aides on an as-needed basis.
  • Monitor and document patient vital signs, administer medications, and provide spiritual support.
  • Educate family caregivers on patient care and support.
Requirements:
  • Licensed Registered Nurse (RN) through the state of Michigan.
  • Hospice experience required.
  • Ability to travel to Macomb, Wayne, and Oakland counties.
  • Commitment to 3 full days of 8-hour work per week.
  • Valid Driver's License and Auto Insurance.
  • Full COVID-19 Vaccination.
  • Current CPR Card.
  • Background check and fingerprinting required.
