As a Family Nurse Practitioner or Physician Assistant, you will be part of our diverse team, committed to cultivating an environment where everyone can thrive. Key Responsibilities:
  • Manage a dynamic patient panel with varied needs;
  • Conduct a mix of acute, chronic, and well visits, treating patients in-office or testing centers, and occasional tele-health visits;
  • Participate in weekly Clinical Rounds and ongoing collaboration with in-office teammates via daily huddles and virtual clinical teams;
Requirements:
  • A completed accredited FNP or PA program with national certification;
  • At least 2 years of experience as an Advanced Practitioner in an outpatient primary care setting seeing patients of all ages (0+); OR 1 year in an urgent care setting coupled with either a 1-year primary care fellowship or 1+ year in an urgent care setting;
  • New York state licensure required upon hire;

  • Demonstrated passion for human-centered primary care;
  • Excellent communication skills to effectively engage with individuals from diverse backgrounds;
  • Clinical proficiency in evidence-based primary care, with adaptability to navigate uncertain situations;
  • Ability to use technology to deliver high-quality care;

Estimated Salary:

$142,000 to $151,000 per year based on a full-time schedule, depending on practice experience and patient care schedule.
