**Job Summary**
We're seeking a skilled Admissions Nurse to join our team. As a key member of our healthcare team, you'll be responsible for completing the admission process, including assessing, planning, implementing, and evaluating patient care plans.
**Key Responsibilities:**
* Welcome patients and assist with the move-in process
* Deliver Welcome Packets, answer questions, and begin the C.A.R.E.S. process
* Coordinate communication with families, nursing home staff, social workers, and team members regarding the admission process
* Complete initial plans of care in the Electronic Medical Record (EMR)
* Conduct head-to-toe assessments and ensure all necessary treatments and orders are in place
* Add wounds to the weekly wound log and initiate preventative measures
* Complete admission paperwork, including initial plans of care, medication lists, admit papers, physician orders, and code status papers
* Verify orders with physicians and obtain and enter patient immunization status into the EMR
* Offer, administer, and document immunizations as needed
* Demonstrate familiarity with agency policies and regulatory requirements
**Requirements:**
* Graduate of an accredited school of nursing
* Bachelor's and/or Master's degree preferred
* Current nursing license in good standing in the state of employment
* Minimum two years of varied work experience as a professional nurse, preferably in a post-acute setting
* Demonstrated skill in administrative performance, including expertise in written and oral communication and documentation requirements
* Ability to assess and respond to patient and family needs in various settings
* Manual dexterity and skillful use of hands when working with patients and equipment
