Primary Care Provider Opportunity

Parker Jewish Institute for Health Care and Rehabilitation is seeking a skilled Primary Care Provider to join our team. As a Primary Care Provider, you will be responsible for assessing members with multiple co-morbidities residing in the community. About the Role

This is a key clinical role within our growing community program. You will perform comprehensive evaluations of patients and provide medical management. Your strong communication and follow-up skills will enable you to effectively work with patients, families, and the interdisciplinary team to create appropriate and viable care plans.

Responsibilities

* Examine patients and order appropriate diagnostic workups
* Assess members with acute change in condition in a timely manner
* Maintain complete and accurate medical records
* Consult with other members of the interdisciplinary team to create appropriate and viable care plans
* Educate patients in topics such as disease process, medication, and compliance
* Medical management of patients

Requirements

* Licensure and current registration to practice as a Nurse Practitioner in New York State
* Strong communication and follow-up skills
* Knowledge of Microsoft, including Excel, and data entry, documentation, and report generation
* Prior experience with the geriatric population and home care strongly preferred
* Knowledge of medical documentation
* Fully vaccinated against COVID-19
