Walker Methodist Westwood Ridge is a premier senior living community that provides comprehensive services to its residents. We are currently seeking a RAI Coordinator for our Transitional Care Unit (TCU) department. This pivotal role involves overseeing documentation and reimbursement processes for residents utilizing Inclusive Insurance and Medicare.

Key Responsibilities:
  • Monitor and ensure accurate charting and reimbursement for residents under Inclusive Insurance and Medicare.
  • Prepare admission paperwork and verify that the Electronic Medical Record is completed accurately by the designated interdisciplinary team member.
  • Serve as the primary trainer for the MDS and care planning procedures.
  • Assist in the completion of MDS assessments as directed by the supervisor.

Qualifications:
  • Current experience with MDS/CAA completion, care planning, and error follow-up.
  • Knowledge of Medicare/PDPM requirements.
  • Experience with ICD-10 coding.
  • Ability to collaborate with the corporate finance team and insurance providers regarding billing issues and claims.

Education:
Graduate of an accredited RN/LPN nursing program with an active RN/LPN license in the State of Minnesota.

Required Experience:
Previous experience in long-term care settings is essential. LPN candidates must possess a minimum of two years of relevant experience.

Preferred Experience:
Familiarity with ICD-10 coding is advantageous.

  • Ability to collect, organize, and analyze data effectively.
  • Strong independent work ethic with excellent time management skills, while also being a collaborative team player.

Major Accountabilities:
  • Ensure the completion of the RAI process for assigned residents.
  • Maintain professional competency through ongoing education, including webinars and conferences.
  • Exhibit professional and courteous behavior towards all stakeholders, including residents, families, and staff.
  • Attend mandatory staff meetings and training sessions as required by federal, state, and organizational standards.
  • Adhere to all relevant regulations and policies, ensuring compliance with the Minnesota Board of Nursing standards.
  • Maintain confidentiality and compliance with privacy regulations concerning residents and employees.
  • Provide training and support to new and existing staff on the RAI process and related software.
  • Engage in quality improvement initiatives for the community and organization.
  • Communicate effectively with supervisors and relevant parties regarding important updates.
  • Complete MDS, CAAs, and care plans in accordance with applicable regulations.
  • Perform additional duties as assigned by the supervisor.
