Job Summary:

The Social Services Designee is a key member of the Front Porch team, responsible for developing and implementing a balanced program of recreational activities and overseeing social service needs for residents at the Health Center.

Key Responsibilities:

  • Provides quality customer service to residents, families, co-workers, and vendors to ensure satisfaction.
  • Designs, plans, and executes diversified schedules of recreational and social activities to promote whole person wellness.
  • Assesses each resident upon admission for recreational and psychosocial needs and develops an individualized plan of care.
  • Provides on-going activity and psychosocial assessments of each resident and revises care plans as necessary.
  • Ensures proper documentation of all assessments for each resident.
  • Attends and contributes to Care Conferences, Risk Management, and QA process and meetings.
  • Works closely with staff to encourage support for programming and involves other staff members as appropriate.
  • Provides psychosocial support and meets non-medical needs for residents and their families.

Requirements:

  • Bachelor's degree in behavioral science or minimum 2 years of experience as a Social Services Assistant providing full range of social services to residents of a geriatric facility.
