Job Summary

We are seeking a skilled Physical Therapist to join our growing team. As a Physical Therapist with GREATLAND HOME HEALTH, you will have the opportunity to work with patients in the home setting, providing a wide range of physical therapy services to help them achieve their maximum potential.

Key Responsibilities
  • Conduct patient assessments to identify physical therapy needs and develop personalized plans of care
  • Deliver high-quality physical therapy services to patients in the home setting
  • Monitor patient progress and adjust treatment plans as needed
  • Teach patients and their families how to perform exercises and activities to promote independence and mobility
  • Collaborate with other healthcare professionals to ensure comprehensive care and support
Requirements
  • Minimum Bachelor's degree from an accredited Physical Therapy program
  • Active Physical Therapy licensure in the state of Illinois
  • Valid driver's license and current auto insurance
  • Previous experience in home care setting preferred
