Mass General Brigham is seeking a skilled Registered Nurse to join our Home Health team in Hatfield, Massachusetts.
Job Summary:We are looking for a compassionate and experienced Registered Nurse to provide high-quality care to patients in their homes. As a Home Health Registered Nurse, you will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities:- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on disease management.
- Conduct thorough assessments of patients' physical and emotional needs, and develop and implement individualized care plans to address these needs.
- Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care and coordination of services.
- Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and to address any concerns or questions.
- Document patient progress and outcomes in a timely and accurate manner, and participate in quality improvement initiatives to ensure high-quality care.
- Graduate of an accredited school of professional nursing required; BSN preferred.
- Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required.
- Minimum of one (1) year Medical/Surgical nursing experience required; home care/hospice experience preferred.
- Demonstrated excellent verbal and written communication skills and strong interpersonal skills required.
- Demonstrated organizational and time management skills required.
- Current driver's license, a good driving record, and reliable transportation required.
