Clinical Pharmacist Endocrinology Specialist

We are seeking a highly skilled Clinical Pharmacist to join our Endocrinology team at Duke University Hospital. As a Clinical Pharmacist, you will play a critical role in providing clinical pharmacy services to patients with endocrine disorders.
Job Summary:The Clinical Pharmacist will work closely with healthcare providers and staff to develop and implement therapy plans, facilitate medication access, and provide patient education. You will also be responsible for monitoring patients for therapeutic and adverse responses to medication, coordinating patient care, and ensuring continuity of care and access to all medications.
Key Responsibilities:- Participate in clinic and care management activities to provide clinical pharmacy services and oversight for safe use of medication.
- Prescribe and adjust medications as permitted through collaborative practice agreement.
- Screen for drug interactions, therapeutic duplications, omissions, etc.
- Monitor patients for therapeutic and/or adverse response to medication.
- Coordinate patient care and ensures appropriate follow-up, including laboratory monitoring, with other outpatient or inpatient teams.
- Ensure continuity of care and access with all medications.
- Facilitate prescription fulfillment through Duke specialty and retail pharmacies, as appropriate.
- Doctor of Pharmacy Degree OR BS Pharmacy Degree.
- Experience with a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment.
- PGY1 or Pharmacy Practice Residency preferred; with 1+ years of clinical practice experience or experience in managing endocrinology patient populations preferred.
- 2+ years of clinical practice experience or experience in managing endocrinology patient populations preferred.
