Work Schedule:
Monday to Friday, 8:30 AM to 5:00 PM.
Role Overview:
As a Physical Therapist (PT), you will play a vital role in an interdisciplinary team dedicated to delivering compassionate, high-quality care and achieving positive patient outcomes. Your responsibilities will include:
- Providing home-based therapy as directed by the attending physician.
- Documenting patient reactions, changes, and progress accurately.
- Delivering skilled physical therapy to alleviate pain and enhance functionality.
Patient Care:
Your primary focus will be on:
- Relieving pain and restoring function through various physical means, including exercise and massage.
- Reducing the risk of re-hospitalization by proactively engaging with high-risk patients.
- Educating patients and families on disease management and preventive measures.
Family Support:
You will serve as an advocate for patients and their families by:
- Counseling them on managing their conditions throughout the physical therapy process.
- Instructing on the proper use of mobility aids and other assistive devices.
Supervision:
Oversee the therapy services provided by Physical Therapist Assistants (PTAs) and nurse assistants as appropriate.
Compliance:
Ensure all clinical documentation adheres to agency protocols and Medicare/Federal guidelines. Maintain communication regarding scheduling and availability with supervisors.
Professional Development:
Participate in mandatory training and agency meetings to stay current in your field. A minimum of one year of experience in an acute care setting is required.
