Job Summary

We are looking for a dedicated and experienced RN to join our team as a per diem hospice nurse. As a per diem RN, you will provide high-quality, patient-centered care to patients in their homes, focusing on their physical, emotional, and spiritual needs.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, medication administration, and treatments.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Conduct thorough assessments, evaluations, and interventions to ensure optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient care and progress accurately and thoroughly, utilizing electronic medical records.
Requirements
  • Graduation from an accredited and approved nursing program.
  • Minimum one year of recent, verifiable clinical (medical/surgical) experience.
  • Prior home care experience strongly preferred, but not required.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
  • Ability to travel to cases as assigned.
