TwelveStone Health Partners is seeking a highly skilled Registered Nurse to join our team as an Infusion Specialist. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and ensuring the safe and effective administration of biologic medications.
Key Responsibilities:- Perform analysis of patient data, including lab work and patient assessments, and report changes to referring physicians.
- Administer medications, infusion therapy, and other appropriate treatments per prescriber orders.
- Provide patient-centered infusion care and serve as an advocate for patients in navigating the healthcare system.
- Obtain referrals from the intake department to initiate home health visits and schedule visit times with patients/caregivers.
- Document accurate and timely patient assessment findings, physical/psychosocial responses to nursing intervention, and progress towards goals of infusion therapy.
- Bachelor's or Associate degree in Nursing.
- Current, active, and unrestricted RN license in the state of practice and CPR certification (BLS minimum) required.
- Minimum of one year experience providing nursing care required, with infusion experience preferred.
- Telephone, MS Office, and computer/keyboard skills required.
- Previous EMR experience preferred.
- Low nurse-to-patient ratio.
- Holiday pay.
