Key Responsibilities- Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
- Administer medications and treatments as prescribed by physicians, ensuring patient safety and adherence to hospital policies.
- Monitor patient vital signs and report any changes or concerns to the nursing staff.
- Conduct patient education and provide guidance on self-care and disease management.
- Collaborate with other healthcare professionals to ensure seamless patient care and communication.
- Document patient information and maintain accurate records.
- Current State of California Licensed Vocational Nurse License
- BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA)
- LVN - IV Therapy Certification (LVN) within 180 days of hire or transfer into position
- Basic Arrhythmia Course required within 180 days of hire or transfer into position
- High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate
