Join Our Team as a Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at Oak Orchard Health. As a member of our Clinical Care Team, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

About the Role

This LPN position offers a flexible schedule, with Sundays and most major holidays off. You will work closely with our medical providers to assist with direct patient care, maintain efficient processes, and ensure the highest level of patient satisfaction.

Responsibilities
  • Maintain patient flow by preparing patients for their visits with providers.
  • Assist providers in responding to patient needs related to medications, lab results, procedures, and preventative/health maintenance measures.
  • Perform pre-visit planning, utilizing software tools and chart review to anticipate health maintenance and chronic care needs.
  • Prepare patients for provider visits, including height/weight, vitals, med reconciliation, and verify allergies.
  • Perform in-house labs and procedures, such as EKGs and peak flow meter readings.
  • Provide age and patient-appropriate screenings and document/report as necessary.
  • Administer vaccines, injections, and PPDs in accordance with standard nursing protocols.
  • Answer and document phone calls from patients and pharmacies, adhering to HIPAA regulations.
  • Clean exam rooms between patients and stock supplies as needed.
  • Preload new patient clinical chart information and complete paperwork as necessary.
  • Initiate and complete medication prior-authorization requests.
  • Perform family medicine/pediatric ED recalls and machine/POCT controls.
  • Accept deliveries/orders and put supplies away.
  • Document refrigerator/freezer temps twice a day.
  • Check expiration dates of injections, medications, samples, instruments, and supplies.
  • Sanitize instruments after use.
  • Maintain accurate and pertinent documentation in the Electronic Health Record (EHR).
  • Adhere to center infection control policies and keep work areas neat and tidy.
  • Actively participate in Quality Assurance and Performance Improvement (QAPI) activities.
  • Participate in monthly nursing staff meetings and other staff meetings within the organization.
  • Maintain current Relias training and adhere to all Oak Orchard policies and procedures.
Requirements
  • Graduate of an accredited program for Licensed Practical Nurse with a current valid license to practice nursing in New York State.
  • One year of experience in a nursing role preferred.
