We are seeking a highly skilled Registered Nurse to join our team as a Care Manager in Home Health. As a Care Manager, you will be responsible for providing comprehensive care to patients in their homes, working closely with our interdisciplinary team to ensure seamless care coordination.
Key Responsibilities- Perform initial assessments of patients and caregivers to determine the appropriateness of home health services
- Develop and implement individualized patient care plans based on ongoing interpretation of physiological, psychological, and sociological deficits
- Coordinate services with other healthcare providers and document all activities and findings
- Ensure compliance with all local, state, federal, and other regulatory agencies
- Maintain patient confidentiality and adhere to established AdventHealth policies and procedures
- Graduate of an approved school of professional nursing
- Minimum of 1 year of clinical experience in Medical/Surgical nursing
- Current Florida licensure as a Registered Nurse
- Current BCLS
- Valid Driver's License
- 2 years of Home Health Nursing experience
- Bachelor's Degree in Nursing
- IV therapy skills desirable
- Venipuncture skills desirable
