CNS Healthcare is seeking a highly skilled and compassionate Psychosocial Rehabilitation Specialist to join our team. As a key member of our rehabilitation team, you will play a vital role in providing direct rehabilitative and clinical services to adult mental health individuals in a clubhouse setting.
Key Responsibilities- Assist in Providing Direct Services: Work closely with our team to provide supportive, educational, and social recreational services to our members.
- Develop and Manage Transitional Employment Placements: Create and manage job opportunities for our members to help them achieve their employment goals.
- Provide Onsite Crisis Intervention and Case Management: Respond to crisis situations and provide case management services to our members.
- Act as Intake Coordinator: Coordinate the intake process for new members, ensuring that all necessary documentation is completed and authorizations are in place.
- Liaise with Internal CNS Case Management Team: Collaborate with our case management team to ensure seamless communication and coordination of services.
- Develop Person-Centered Plans: Work with our members to develop personalized plans that meet their unique needs and goals.
- Participate in Quality Improvement Initiatives: Contribute to departmental and agency quality improvement initiatives to ensure that our services are of the highest quality.
- Complete Required Documentation: Ensure that all required documentation is completed in a timely fashion, meeting productivity and direct service time standards.
- Bachelor's Degree in Social Work or Related Field: A bachelor's degree in social work or a related field is required.
- Qualified Mental Health Professional: Must be a qualified mental health professional.
- Experience Working with Adults with Serious Mental Illness: A minimum of one year of experience working with adults with serious mental illness is required.
- Valid Michigan Driver's License and Good Driving Record: Must have a valid Michigan driver's license and a good driving record.
- Basic Understanding of Principles of Psychosocial Rehabilitation: Must have a basic understanding of the principles of psychosocial rehabilitation.
- Basic Understanding of Michigan Mental Health Code and Recipient Rights: Must have a basic understanding of the Michigan Mental Health Code and Recipient Rights.
- Working Knowledge of Applicable Laws and Regulations: Must have a working knowledge of applicable federal, state, and local laws and regulations.
- Ability to Maintain Professional Boundaries: Must be able to maintain professional boundaries.
- Ability to Communicate Effectively: Must be able to communicate effectively, professionally, and courteously.
- Ability to Use Discretion and Judgment: Must be able to use discretion and judgment when handling matters of a sensitive or confidential nature.
