Job Summary

Resilient Healthcare & Home Health is seeking an experienced Advanced Practice Provider to join our team. As a key member of our healthcare delivery system, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Conduct home visits to assess patient needs and develop personalized care plans
  • Perform physical examinations, laboratory testing, and diagnostic procedures as needed
  • Prescribe medications and treatments within the scope of your expertise and protocols
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Participate in health education and promotion activities to empower patients and their families
Requirements
  • Master's degree in Science of Nursing or Physician Assistant Studies
  • Minimum 3 years of advanced practice provider experience
  • Advanced Practice Registered Nurse License or Physician Assistant License in the State of Texas
  • Family Nurse Practitioner or Adult Gerontology or Acute Care Nurse Practitioner certification or Physician Assistant Certification from National Commission on Certification of Physician Assistants
  • American Red Cross or American Heart Association Basic Life Support Course (BLS or BLCLS) and Certification
  • American Red Cross or American Heart Association Advanced Life Support Course (ALS or ACLS) and Certification
Working Conditions

This role requires frequent travel to patients' homes and may involve exposure to adverse environmental conditions, bloodborne pathogens, physical hazards, and chemical hazards.

Equipment Used

This role routinely uses standard office equipment such as computers, keyboards, telephones, fax machines, copy machines, and calculators, as well as medical equipment such as blood pressure cuffs, thermometers, and stethoscopes.

Physical Requirements

This role requires regular standing, walking, reaching, twisting, stooping, and kneeling, as well as the ability to use hands and fingers to handle or feel and to talk or hear.
