Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will play a vital role in delivering high-quality patient care and working collaboratively with our healthcare professionals.

Key Responsibilities
  • Deliver post-acute care to patients, including monitoring vital signs and administering medications
  • Conduct intakes of residents, documenting their health concerns and medical histories
  • Review medical records and charts to ensure accurate and up-to-date information
  • Administer medications and verify their accuracy
  • Maintain a safe and comfortable environment for residents
  • Assist with IV set up, vaccinations, and phlebotomies
  • Support medical staff with orientation and training
Requirements
  • Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of an approved Licensed Practical Nurse (LPN) educational program
  • High School diploma or equivalent
  • Minimum of 1 year of experience as an LPN or in a related role
