Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Diversicare. As a Regional LPN, you will play a vital role in providing high-quality nursing care to our residents in a fast-paced and dynamic environment.

Key Responsibilities

  • Provide direct patient care to residents, including administering medications, performing treatments, and monitoring vital signs.
  • Assess and document resident needs, develop and implement individualized care plans, and collaborate with interdisciplinary teams to ensure comprehensive care.
  • Supervise and mentor Certified Nursing Assistants (CNAs) to ensure high-quality care and adherence to company policies and procedures.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance resident care and satisfaction.
  • Communicate effectively with residents, families, and healthcare teams to ensure seamless care transitions and address concerns in a timely manner.

Requirements

  • Current, unrestricted LPN license in the state of practice.
  • Minimum 1 year of experience in long-term care or post-acute care settings.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Valid driver's license and current auto insurance required.
