Job Summary

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct home risk assessments, medical history reviews, and health education sessions with at-risk health plan members.

You will work collaboratively with our team to provide comprehensive care and improve health outcomes. This is a flexible opportunity with a variety of scheduling options available.

Responsibilities

As a Nurse Practitioner with Matrix Medical Network, your responsibilities will include:

  • Conducting home risk assessments and medical history reviews
  • Providing health education and counseling to at-risk health plan members
  • Collaborating with physicians, case managers, and other healthcare professionals to provide comprehensive care
  • Documenting patient information and care plans

You will also have the opportunity to work in a variety of settings, including home visits, skilled facilities, and telehealth visits.

Requirements

To be considered for this opportunity, you must have:

  • A Master's degree in Nursing or a related field
  • Current RN and NP licensure in the state of practice
  • Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
  • A minimum of 1 year of experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent verbal and written communication skills

You must also be able to travel to meet patients in their homes and have a valid state driver's license and adequate automobile insurance coverage.

What We Offer

We offer a competitive compensation package, including a base rate of $95.00 or $100.00 per completed assessment, depending on the county. We also offer a mileage reimbursement plan and flexible scheduling options.
