For over 15 years, we have been delivering exceptional care throughout southern Wisconsin, maintaining our status as a Joint Commission accredited organization.
The Registered Nurse Case Manager will collaborate closely with our Enterprise Director of Nursing and the Baraboo Branch Manager to ensure high-quality care for our clients. Responsibilities will include conducting initial client assessments, supervising the care of current clients (both personal and skilled care), and coordinating with skilled service organizations and hospices. Key qualities for success in this role include compassion, patience, and an approachable demeanor to foster trust with clients and their families.
Key Responsibilities:
- Deliver care in accordance with the state's nurse practice act.
- Conduct initial evaluations and regularly reassess clients' nursing and personal assistance needs.
- Implement preventive and rehabilitative nursing procedures while providing guidance to clients and families on nursing-related matters.
- Adhere to agency policies and procedures.
- Evaluate staff competency to ensure quality, continuity, and safety in client care.
- Encourage home care referrals within the healthcare community.
Qualifications:
- Must possess a current, unencumbered Registered Nurse license in Wisconsin.
- A minimum of three years of documented nursing experience is required, with at least two years of clinical experience and one year in a home health agency or case management preferred.
- Familiarity with regulatory compliance, including local, state, and federal laws, Joint Commission standards, OSHA, and HIPAA.
- Strong time management skills and flexibility are essential.
- Willingness to travel locally is required.
