We are seeking a compassionate and skilled LPN to join our Hospice team at Chapters Health System. As a Hospice LPN, you will play a vital role in providing high-quality patient care and support to individuals and their families.
Key Responsibilities- Under the direction of an RN, provide nursing tasks to patients and families, including continuous care, dressing changes, and catheter changes.
- Demonstrate clinical skills in medication administration, symptom management, and patient/family relationships.
- Monitor patients for alterations in body systems, pain, and symptoms, and report to RN.
- Perform venipuncture for blood specimens and other duties as assigned.
- A high school diploma or G.E.D. or an equivalent combination of education and experience.
- Current LPN licensure in the state of Florida or current licensure from another state plus a temporary Florida permit.
- Minimum of one (1) year nursing experience in an in-patient, hospice, or home health setting.
- Mobile Driver - Valid driver's license and automobile insurance per Company policy.
- Active BLS for healthcare professionals from the American Heart Association or Red Cross.
