We are looking for a skilled Registered Nurse to join our team at VitalCaring Group. As a member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Your expertise and compassion will make a meaningful difference in the lives of our patients and their families.
Responsibilities:
* Deliver patient-centered care that addresses the physical, emotional, and social needs of patients
* Develop and implement personalized care plans that meet the unique needs of each patient
* Collaborate with the care team to ensure seamless communication and coordination of care
* Educate patients and their families on self-care techniques and disease management strategies
Qualifications:
* Current RN license and valid state driver's license
* One year of experience as a Registered Nurse in an acute or clinical care setting preferred
* Home health experience a plus
