Job Summary

We are seeking a highly motivated and compassionate Street Outreach Advocate to join our team at Northwest Indiana Community Action. As a key member of our outreach team, you will play a vital role in connecting individuals and families experiencing homelessness with essential services and resources.

Key Responsibilities
  • Conduct daily outreach activities to engage with individuals and families in need, providing crisis intervention services and referrals as necessary.
  • Assess the needs of homeless individuals and families, coordinating with local resources to address those needs and facilitate access to emergency shelter, permanent housing, and supportive services.
  • Develop and implement individual service plans to help clients achieve stable housing, conducting regular follow-ups to monitor progress and re-evaluate needs.
  • Collaborate with local agencies, law enforcement, and community groups to enhance service delivery and outreach effectiveness.
  • Advocate for the needs and rights of homeless individuals at various forums and policy discussions.
Requirements
  • Valid driver's license and reliable transportation; willingness to travel extensively across designated regions.
  • Ability to navigate various terrains and weather conditions during outreach activities.
  • Flexible schedule required, including evenings, weekends, and holidays to accommodate outreach needs.
