Job Title: Physical Therapist - Outpatient

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in the Fort Collins area. As an outpatient Physical Therapist, you will work with clients in independent living, assisted living, and memory care communities, providing high-quality care and support.

Responsibilities:
  • Make outpatient visits to clients in designated senior living communities.
  • Supervise Physical Therapy Assistants as necessary or applicable.
  • Attend and contribute to patient care, staffing conferences, and other related meetings.
  • Make referrals to other disciplines to address comprehensive patient needs.
Requirements:
  • A current CO Physical Therapist license.
  • Outpatient experience a plus but not required.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Dynamic and strong interpersonal, organizational, and problem-solving skills.
