About the Role

As a Nurse Practitioner at Abode Care Partners, you will provide compassionate evidence-based medical care to patients with complex needs. Your responsibilities include evaluating and treating illnesses, collaborating with healthcare professionals, and maintaining up-to-date patient records.

Key Responsibilities:

  • Evaluating and treating chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses
  • Collaborating with caregivers and other healthcare professionals to deliver interdisciplinary care and maintain a comprehensive plan of care
  • Providing referrals to other healthcare practitioners when necessary
  • Maintaining accurate and up-to-date patient medical records

Requirements:

  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
