Core Responsibilities

  • Assess and review physician medication orders for appropriateness, including drug selection, dosage, potential interactions, and administration routes. Provide recommendations for therapy adjustments as necessary.
  • Maintain a comprehensive patient database, ensuring accurate documentation of medical history, clinical data, and current medications.
  • Order and evaluate necessary laboratory tests for monitoring high-risk conditions, collaborating with healthcare team members to discuss findings.
  • Work alongside the healthcare team to provide clinical and technical support as required.
  • Document all communications with healthcare professionals and patients in the medical record accurately.
  • Implement and uphold policies and procedures for the safe handling, storage, distribution, and disposal of pharmaceuticals.
  • Ensure compliance with all regulatory requirements and maintain necessary documentation.
  • Report any medication-related errors to the Pharmacy Manager promptly.
  • Assist facility staff with procedures related to the safe procurement and administration of medications.
  • Perform additional tasks as assigned by management.


Qualifications

  • Bachelor's Degree in Pharmacy; Doctor of Pharmacy (Pharm D) preferred.
  • Minimum of one year of experience in a long-term care setting.
  • Licensure and certifications as mandated by regulatory agencies.
  • Detail-oriented, goal-driven, and a collaborative team player with strong communication skills.
