Key Responsibilities
As the RN Case Manager, you will take full nursing responsibility for implementing the Plan of Care for each hospice patient, while continuously assessing both personal and professional performance to enhance productivity and quality of care provided.
- Conduct the initial nursing assessment to determine eligibility for hospice services during a visit within forty-eight (48) hours of referral.
- Identify the physical, psychosocial, and environmental needs of the patient/family and reassess as necessary, at least every fifteen (15) days.
- Initiate and coordinate the care plan while maintaining precise and relevant clinical documentation regarding the patient's condition.
- Record issues, set appropriate goals, interventions, and document patient/family responses to hospice care.
- Collaborate with the patient/family, attending physician, and other members of the interdisciplinary group in providing daily care for the patient and family.
Experience Requirements
- A minimum of one year of clinical experience.
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR Certification.
- Valid driver's license, vehicle insurance, and access to reliable transportation, or public transit options.
