Job Summary

We are seeking a compassionate and skilled Pediatric Nurse/Home Health Care RN to join our team. As a member of our team, you will provide one-on-one care to pediatric patients in their homes, utilizing your clinical expertise to create a safe and nurturing environment.

Responsibilities
  • Provide skilled nursing care to pediatric patients in their homes, including medication management, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
  • Document patient progress and communicate with families and healthcare teams to ensure informed decision-making.

Requirements
  • Current BLS CPR certification, obtained in-person.
  • Alzheimer's training, including a 2-hour DOEA-approved course and a 1-hour DOEA video.
  • Ability to work independently and as part of a team to deliver high-quality patient care.
  • Strong communication and interpersonal skills to effectively collaborate with families and healthcare teams.
  • Ability to adapt to changing patient needs and priorities.
