Home Health Physical Therapist Role Overview

We are seeking a motivated and compassionate Physical Therapist (PT) to join our team at Interim HealthCare of West Palm Beach Careers, serving the Vero Beach and Sebastian areas. As a member of our home health team, you will have the opportunity to make a meaningful difference in the lives of patients who require personalized therapy services.

About the Job

This rewarding role involves providing high-quality, patient-centered physical therapy services in a home-based setting. You will work collaboratively with a multidisciplinary team, including nurses, occupational therapists, and speech-language pathologists, to develop and implement comprehensive care plans that cater to each patient's unique needs and goals.

Key Responsibilities:

  • Conduct thorough assessments and evaluations to identify patients' physical limitations and develop effective treatment plans.
  • Deliver individualized physical therapy interventions, including exercises, stretches, and modalities, to help patients regain strength, mobility, and function.
  • Educate patients and their families on proper techniques, safe practices, and self-care strategies to promote optimal recovery and independence.
  • Maintain accurate and timely documentation of patient progress, treatments, and outcomes.
  • Collaborate with the interprofessional team to ensure seamless communication and coordination of care.

Requirements and Qualifications

To be considered for this position, candidates must possess:

  • A graduate degree from an accredited Physical Therapy Program and an active PT license in Florida.
  • A minimum of one year of experience in home healthcare or a related setting.
  • Current CPR certification.
  • Familiarity with state and federal home health regulations.
  • Excellent clinical judgment, strong interpersonal skills, and compassion.
