BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Harrisburg, Pennsylvania.
About the Role:As a Physical Therapist with BAYADA, you will be responsible for providing high-quality, patient-centered care to adult and geriatric clients in their homes. You will work closely with our multidisciplinary team to develop and implement individualized care plans, conduct diagnostic tests and measurements, and educate clients and their families on rehabilitative care and activities.
Responsibilities:- Conduct diagnostic tests and measurements to assess client needs
- Develop and implement individualized care plans to promote client independence and safety
- Provide manual therapeutic exercises, gait training, and other interventions as needed
- Document client progress and revise care plans as necessary
- Collaborate with nursing and other healthcare professionals to ensure comprehensive care
- Current Pennsylvania Physical Therapist license
- Minimum one year of recent work experience as a Physical Therapist
- Graduation from a program approved by the American Physical Therapy Association
- Solid computer skills and prior experience with electronic medical records (EMR) preferred
