Job Title: Certified Nurse Aide

At Carriage House Nursing and Rehab, we are seeking a dedicated and compassionate Certified Nurse Aide to join our team.

Job Summary:

The primary purpose of this role is to provide high-quality nursing care to our residents, ensuring their physical, emotional, and social needs are met.

Key Responsibilities:
  • Nursing Care Functions:
    • Record accurate and informative entries on flow sheets, notes, and charts.
    • Report all accidents and incidents to the Charge/Staff Nurse.
    • Monitor and report changes in resident condition to the Charge/Staff Nurse.
    • Perform and/or assist residents with hygienic and grooming functions.
    • Change bed linens and make beds.
    • Collect and report specimens as instructed.
    • Check residents' personal care needs and report any concerns.
    • Lift, turn, move, position, and transport residents safely.
    • Assist residents with walking, using self-help devices, and rehabilitative care.
    • Provide post-mortem care as instructed.
    • Observe and report skin breakdown and decubitus ulcers.
    • Provide daily indwelling catheter care and perineal care.
    • Perform daily Range of Motion Exercises.
    • Turn bedfast residents every two hours or as directed.
    • Provide social interactions and engage residents in activities.
  • Admission, Transfer, and Discharge Functions:
    • Prepare resident rooms for admission, transfer, and discharge.
    • Participate in admission, transfer, and discharge functions according to Facility policy.
  • Food Service Functions:
    • Prepare residents for meals, including taking them to the bathroom, washing hands, and combing hair.
    • Serve food trays and assist with feeding as needed.
    • Record residents' food and fluid intake and report changes in eating habits.
    • Perform after-meal care, including removing trays and cleaning residents' hands and face.
    • Check rooms for unauthorized food items.
  • Staff Development Functions:
    • Attend and participate in scheduled training and educational classes.
    • Attend and participate in scheduled orientation programs and activities.
    • Wear a name tag on your clean scrub uniform and refrain from wearing artificial nails.
  • Safety and Sanitation Functions:
    • Notify the Charge/Staff Nurse of any resident leaving or missing from the Facility.
    • Follow established Infection Control and Universal Precautions policies and procedures.
    • Keep residence living units clean and safe.
    • Report any contagious diseases to the Director of Nursing Services and/or Infection Control Coordinator.
    • Provide illness symptoms when calling in sick to track and prevent illness.
    • Ensure resident safety.
  • Equipment and Supply Functions:
    • Report defective equipment to the Charge/Staff Nurse.
    • Inform the Charge/Staff Nurse of equipment and supply needs.
  • Patient Care In-Put Functions:
    • Inform the Charge/Staff Nurse of any changes in resident condition for accurate care plan entry.
    • Review daily nursing assistant assignments and Kardex instructions for assigned residents.
  • Resident Rights Functions:
    • Maintain confidentiality of all resident information.
    • Never take pictures inside the Facility without permission from the Administrator.
    • Treat all residents fairly, with kindness, dignity, and respect.
    • Report all grievances and complaints made by residents to the Nurse in Charge.

Working Conditions:

  • Works throughout the Facility.
  • Sits, stands, bends, lifts, and moves during working hours.
  • Subject to frequent interruptions.
  • Involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
  • Subject to hostile and emotionally upset residents, family members, personnel, and visitors.
  • Works beyond normal working hours, weekdays, weekends, and holidays when necessary.
  • Subject to odors throughout the workday.
  • May be exposed to infectious waste, diseases, conditions, etc., including the potential exposure to the AIDS and Hepatitis B viruses.
  • Must be able to lift, push, pull, and move objects and patients to the extent necessary to perform the functions listed above.
  • May be required to assist in the evacuation of residents.

Requirements:

  • Must be a licensed Certified Nurse Aide in accordance with laws of this state.
  • Must be able to read, write, and speak the English language in an understandable manner.
