About the RoleAs a Primary-Care Provider, you will work in coordination with an LTC Collaborating Physician to deliver comprehensive care to patients. Your responsibilities will include day-to-day care, patient assessment, and development of individualized care plans.
Requirements and Qualifications- Experience in Family Medicine, Internal Medicine, or Geriatrics is preferred, but new graduates are also welcome to apply.
- Candidates must hold a valid and unrestricted NP medical license in Michigan.
- Candidates must have a valid DEA license in the state of Michigan.
- Candidates must have completed a valid U.S. certification program.
- This is an in-person position, with providers commuting directly between home and facilities.
- After-Hours on-call is shared among all LTC Providers.
- Productivity-Based Compensation Model (No Cap)
- $10k SIGN ON BONUS
- 15 PTO days annually
- $2,500 Annual CME allowance plus 3 PTO days to use for CME attendance
- Company-paid subscription to Up To Date, both mobile and laptop
- Monthly phone stipend
- All medical licensure expenses reimbursed by LTC
- Paid professional liability-malpractice insurance with tail coverage
- 401k with employer matching (after 90 days of employment)
