We are seeking a compassionate and skilled LPN to join our team at The Moments, a leading provider of memory care and assisted living services. As an LPN, you will play a vital role in ensuring the health and safety of our residents, leading and directing your care team, and collaborating with residents, families, and healthcare professionals to create personalized care plans.
Key Responsibilities:
- Ensure the health and safety of residents through effective care planning and delivery.
- Lead and direct your care team to provide high-quality care and support to residents.
- Collaborate with residents, families, and healthcare professionals to create person-centered care plans that enhance the lives of our residents.
- Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
- Prioritize and organize work effectively to meet the needs of our residents.
Requirements:
- Current licensure as an LPN in the State of Minnesota.
- Ability to work with a team to provide high-quality care and support to residents.
- Excellent communication and interpersonal skills.
- Ability to prioritize and organize work effectively.
