**Job Summary:**
As an LPN, you will play a vital role in providing direct patient care under the supervision of a registered nurse. Your responsibilities will include following the plan of care, providing treatments, and working collaboratively with the healthcare team to achieve positive patient outcomes.
**Key Responsibilities:**
• Provide direct patient care as defined in the state Nurse Practice Act.
• Implement the plan of care initiated by the registered nurse.
• Document patient information accurately and timely, consistent with the plan of care.
• Assess and educate patients and their families/caregivers on diagnosis and plan of care.
• Assist the registered nurse or physician in performing specialized procedures and duties, including preparing equipment and materials for treatments, observing aseptic technique as required, and assisting patients in learning self-care techniques.
**Qualifications:**
• Graduate of an accredited practical nurse or vocational nursing program.
• Preferably 3 years of nursing experience, with community/home health or medical/surgical experience being a plus.
• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
• Comply with accepted professional standards and practices.
• Possess and maintain CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
