We are seeking a skilled Nurse Practitioner to provide high-quality, patient-centered care to residents in skilled nursing facilities (SNFs) across the country. As a Primary Care Provider, you will be responsible for rounding and managing the clinical care of patients, prescribing medications and treatments, and providing comprehensive primary care services.
### Key Responsibilities
* Round daily to assess patient needs and provide care
* Prescribe medications and treatments as needed
* Provide high-quality, patient-centered care to patients in SNFs
* Document patient care and encounters in a timely manner
* Be available for phone and secure messaging consultations with facility nursing staff
### Work Environment
* Work in a fast-paced, dynamic environment with a focus on patient care
* Collaborate with facility nursing staff and other healthcare professionals to provide comprehensive care
* Use electronic health records (EHRs) to document patient care and communicate with colleagues
