Job Overview

A dedicated individual who is specifically trained and skilled in providing essential personal care services to patients in their homes under the guidance and supervision of a registered nurse (RN).

Qualifications
  • Must fulfill Medicare Conditions of Participation for Home Health Aide training and competency.
  • Possess a compassionate attitude towards the care of the ill and elderly.
  • Ability to follow instructions and communicate effectively.
  • Maturity and capability to manage the responsibilities of the role.
Key Responsibilities
  • Comprehend and comply with established Agency policies and procedures.
  • Provide personal care, bathing, and hands-on assistance as required.
  • Complete necessary visit documentation promptly in accordance with Agency standards.
  • Notify the RN of any changes in the patient's condition and requirements.
  • Perform household tasks essential to health care in the home as directed.
  • Assist with patient mobility and exercise as instructed.
  • Carry out simple procedures as an extension of therapy or nursing services, such as range of motion (ROM) exercises as assigned.
  • Support with medications that are typically self-administered as directed.
  • Participate in in-service and continuing education programs as needed.
  • Attend patient care meetings as scheduled.
Work Environment
Work is conducted indoors in Agency offices and patient residences, with travel to and from patient homes.

Ability to perform the following tasks if necessary:
  • Engage in physical activity.
  • Stand and be active for extended periods.
  • Perform heavy lifting.
  • Engage in frequent bending, lifting, and standing.
