As a key member of our team, you will play a vital role in creating a comprehensive system of care for heart failure patients, focusing on high-quality and efficient hospital care, population health management, and identification of patients needing advanced procedural treatments.

You will work closely with multidisciplinary teams, including primary care, cardiovascular specialists, and procedural teams, to deliver exceptional patient care.
