Job Title: Registered Nurse

Responsibilities:
  • Provide skilled nursing care to patients in their homes, adhering to physician orders and developing individualized care plans.
  • Conduct thorough assessments of patients' conditions and needs, documenting all activities, assessments, and care provided.
  • Develop, implement, and update nursing care plans to achieve expected outcomes and improve patient quality of life.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
  • Administer prescribed medications, treatments, and therapies, ensuring patient safety and comfort.
  • Provide education and support to patients and families on disease management, self-care techniques, and prevention strategies.
Requirements:
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current, unrestricted state license as a Registered Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
