We are seeking a highly skilled and compassionate Care Promoter, Specialist Podiatry to join our team at ChenMed. As a key member of our Physician-led Care Team, you will play a vital role in providing exceptional patient care and support to our patients.
Key Responsibilities- Measure and record patient vital signs, and document patient interview and medical history.
- Collaborate with the care team to ensure specialist-related medical records and consult notes are obtained.
- Prepare patients for examinations and perform routine screening tests, assist physicians with exams, and explain treatment procedures and physicians' instructions to patients.
- Escort patients to exam rooms and ensure their comfort and well-being prior to leaving.
- Observe patients and report any changes in patient conditions to the Specialist, PCP, Clinician, and/or other clinical personnel.
- Perform non-critical medical and therapeutic procedures based on medical instructions, collect phlebotomy and other lab specimens, and perform basic waived lab tests.
- Prepare and administer medications, change dressings, apply bandages, remove sutures, and perform other first aid procedures under physician supervision.
- Call in prescriptions to the pharmacy upon physician approval and authorization.
- Use communication skills with appropriate medical terminology and follow legal and ethical professional conduct.
- Provide health coaching to a defined group of patients to support healthy lifestyle choices and follow up with coached patients via weekly calls.
- Catalog and communicate patient needs to the Specialist, PCP, Clinician, and/or other clinical personnel.
- Maintain supplies, equipment, stocks, and sterilize instruments, practice OSHA safety standards, and perform accurate, legal, and ethical documentation at all times.
- Work in conjunction with care facilitators to greet patients, schedule appointments, and ensure all patients have follow-up appointments with PCP and Specialist.
- Work in conjunction with care coordinators to ensure Specialist orders are completed and patients are scheduled accordingly.
- Present patients with customer service surveys during check-out and escalate if needed for immediate service recovery.
- Process patient phone messages, return calls, and route them to other team members as appropriate, and call patients to obtain and relay pertinent information for the physician.
- High school diploma or equivalent education (GED) required.
- Certified Medical Assistant from a nationally recognized certifying agency required.
- A minimum of 1 year of work experience as a Medical Assistant, Patient Care Technician, or similar in a hospital, medical clinic, or comparable environment required.
- Experience working with geriatric patients is a plus.
- Experience working with Cardiologist, Podiatrist, or other specialists preferred.
- EMR system experience preferred.
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within the first 90 days of employment.
