Your Impact:

Arkos Health is in search of a Physician's Assistant - Home Wellness. The successful candidate will exhibit a high level of clinical proficiency in managing patients with both acute and chronic health issues. Responsibilities include assessment, diagnosis, treatment planning, management, education, health promotion, and care coordination tailored to member health needs. This role requires a valid Physician's Assistant certification and adherence to state laws and regulations regarding practice scope.

Key Responsibilities:

  • Conduct thorough in-home health assessments that are complete, accurate, and appropriate for the patient's age, gender, and clinical condition.
  • Possess a deep understanding of the causes, risk factors, and clinical presentation of various health conditions.
  • Review patient records prior to visits to ensure accurate diagnosis and treatment planning.
  • Develop, implement, and adjust treatment plans as necessary.
  • Make informed recommendations regarding medication adjustments.
  • Maintain precise and timely documentation in the electronic health record, including:
    • Correct diagnosis codes
    • Status updates on all identified health conditions
    • Plans for addressing each diagnosis noted during visits
  • Demonstrate knowledge of Risk Adjustment Factor (RAF) and Hierarchical Condition Categories (HCC) models within the first month of employment.
  • Understand and apply Quality STAR/HEDIS measures within the first month of employment.

Qualifications:

  • Certification as a Physician's Assistant
  • Unrestricted prescribing and dispensing privileges with DEA licensure
  • Preferred: 2 years of experience as a Physician's Assistant
  • Preferred: Experience with adult and geriatric populations
  • Preferred: Familiarity with population health management
  • Strong problem-solving skills and ability to prioritize patient needs
  • Flexibility and adaptability to changing organizational demands
  • Valid driver's license and proof of insurance required
