Job Title: Registered Nurse - Home Health

Sharon Regional Health System Home Health is seeking a skilled and compassionate Registered Nurse to join our team. As a Registered Nurse in Home Health, you will provide high-quality patient care in the comfort of their own homes.

Key Responsibilities:
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Conduct comprehensive assessments, including medication reconciliation, to ensure accurate and timely documentation.
  • Provide education and support to patients and families to promote health and wellness.
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions.
  • Document patient visits and progress accurately and thoroughly, adhering to agency policies and payer requirements.
Requirements:
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
Preferred Qualifications:
  • One year of prior professional nursing experience.
  • Experience working in home health or a similar setting.
