LHC Group is seeking a skilled Physical Therapist to join our team in Huntingdon, Tennessee.
About the RoleWe are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to our patients in their homes. As a Physical Therapist with LHC Group, you will have the opportunity to work with a diverse range of patients and develop a personalized treatment plan to meet their unique needs.
Responsibilities- Evaluate patient functional status and physical therapy needs, assisting the physician in developing a therapy plan of care.
- Develop and implement treatment plans to improve and enhance patient well-being, including exercises, stretches, and other interventions.
- Monitor patient progress and report any changes in condition to the supervising nurse and/or physician.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- Access to a dependable vehicle and valid driver's license.
