Key Responsibilities- Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
- Provide professional nursing care, including administering medications, wound care, and other treatments as needed.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Current licensure as a Registered Nurse in the state.
- At least one year of acute care experience in an institutional setting.
- One year of experience in home care preferred.
- Ability to travel throughout the Bristol Hospice Service Area.
