Cahaba Medical Care Foundation is seeking a skilled and compassionate Chronic Care Management Nurse RN or LPN to join our team.
Job Summary:The successful candidate will be responsible for assisting with Population Health Management, including calling patients weekly, reviewing medications, and patient goals. Additionally, the nurse will be responsible for scheduling patients, managing patient lists, and performing billing for visits.
Key Responsibilities:- Assist with QA projects between patient calls
- Triage patients prior to physician encounters, including obtaining vital signs and completing intake forms
- Administer injections as ordered by physicians or nurse practitioners
- Prepare examination rooms and supplies for procedures
- Assist with patient paperwork, including patient assistance and FMLA paperwork
- 2-year degree as a RN or LPN
- Three to five years of experience in an outpatient clinical setting
- BLS certification
- 401K
