Key Responsibilities

You will independently assess patients, formulate a treatment plan based on targeted objectives, deliver and document care, and facilitate discharge planning for all physician-referred patients. You will supervise and guide the activities of support staff. All tasks must be performed in accordance with the professional code of ethics, standards of practice, state and national regulations, hospital standards, and departmental policies and procedures. Additionally, in the absence of the Lead PT, you may be required to coordinate workflow within the Home Health therapy department, including planning daily staffing and coverage schedules.

Qualifications

AHA BLS for Healthcare Providers certification within 90 days. Auto liability insurance. Clean driving record. Valid driver's license. Current Physical Therapy licensure in the state of practice. Minimum of two years of relevant experience as a physical therapist with a satisfactory proficiency examination score. Successful completion of KDADS criminal record check upon hire. Registration and successful completion of the Family Care Safety Registry upon hire.
