About the Role

We are seeking a skilled Speech-Language Pathologist to join our team at EmpowerMe Wellness. As a key member of our care coordination program, you will have the opportunity to practice at the top of your license while contributing to the growth and development of our innovative program.

Responsibilities
  • Provide speech-language pathology services to older adults in a senior living community setting
  • Conduct assessments, treatment planning, and therapeutic interventions as part of a multidisciplinary team
  • Support care coordination efforts by conducting regular wellness checkpoint visits, communicating with patients' providers and caregivers, and assisting patients with attending telehealth visits
  • Maintain professional practices and ethical standards in accordance with company-wide policies and clinical initiatives
  • Contribute to program development, quality improvement, and problem-solving

Requirements
  • Completion of an accredited Speech-Language Pathologist program
