Role Description:
During a visit that may last up to one hour, Matrix providers assess and evaluate a member's current health status, medical history, medication compliance, social circumstances, and other potential risks. Key Responsibilities:
Role Expectations:

Nurse Practitioner - Home Health Assessments
Type: PRN - Flexible - Nurse Practitioner
Location: Various community settings including home visits, skilled nursing facilities, and telehealth.
Hours: Flexible scheduling available including days, evenings, nights, and weekends.
Compensation: $90.00 or $95.00 per completed assessment, with a minimum of 25 hours or more per month.
Provider Expectations:
- Make a meaningful difference in patients' lives by conducting assessments, reviewing medical histories, diagnosing and treating conditions, providing health education, and coordinating referrals.
- Collaborate effectively with physicians, case managers, social workers, family members, and other relevant healthcare personnel.
- Engage with Primary Care Physicians (PCPs) for patient education and follow-up care.
- Build and maintain relationships within a nationwide network of advanced practice providers.
- Deliver services in various settings, including home visits, skilled facility visits, and telehealth consultations.
Qualifications:
Essential Requirements:
- Master's Degree or equivalent experience with successful completion of NP licensure.
- Current RN and NP licensure in the state of practice, including prescriptive authority or the ability to obtain it.
- Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- A minimum of one year of experience as a Nurse Practitioner.
- Proficient computer skills and familiarity with electronic health record systems.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- Ability to adapt to frequent changes in a dynamic environment.
Travel Requirements:
- Must possess a valid state driver's license and have the ability to travel, including proof of adequate automobile insurance coverage.
- Travel may be necessary.
