We are seeking a highly skilled Clinical Pharmacist to join our team at CHRISTUS Health. As a Clinical Pharmacist, you will play a critical role in providing exceptional patient care and contributing to the success of our healthcare organization.
Key Responsibilities- Prepare and dispense medication orders according to established policies and procedures.
- Interpret and transcribe medication orders, maintaining accurate and complete patient medication records.
- Compound and dispense pharmaceuticals, including non-sterile and sterile products, chemotherapy, and parenteral nutrition products.
- Issue controlled substances to patient care areas and maintain records as required by law and institutional policies.
- Provide clinical consultation and clarification to practitioners as needed.
- Participate in quality improvement and medication use review activities, collecting data and conducting monitors and inspections.
- Conduct designated interventions, such as IV to PO conversions and renal dosing support.
- Participate in antibiotic stewardship, opioid stewardship, and shortage management activities.
- Doctor of Pharmacy or BS Pharmacy degree required.
- Hospital experience preferred.
- Pharmacy registration in the State of practice.
- Must have pharmacy State licensure within 60 days of start date.
- BLS or ACLS and PALS (pediatric practice) within 180 days of start date.
This position requires the ability to work in a fast-paced environment and to prioritize tasks effectively. The Clinical Pharmacist will work closely with other healthcare professionals to provide exceptional patient care and to contribute to the success of our healthcare organization.
